The simplest Ujjain Omkareshwar Maheshwar Mandu itinerary is a five-night, six-day loop: two nights in Ujjain, one at Omkareshwar, one at Maheshwar and one in Indore after Mandu, leaving on day six. It is close to MP Tourism's own plan, but splits the two Narmada nights between Omkareshwar and Maheshwar. The longest drive is Ujjain to Omkareshwar.

These four places sit in a rough arc across western Madhya Pradesh: Ujjain, then south to Omkareshwar on the Narmada, west along the river to Maheshwar, and on to the fort city of Mandu before returning to Indore. Indore is the natural start and end because it has the airport, as Wikipedia notes for Omkareshwar, and it is close to every stop. Why this order works
The loop runs in one direction, so you never backtrack over the same road. Ujjain comes first because it is the closest to Indore, about 55-60 km and roughly 1.5 hours, which means you can reach it on the day you land. From Ujjain the road to Omkareshwar is the longest single drive of the trip, so it gets its own day. After Omkareshwar the Narmada leads you to Maheshwar, only about 65 km and 1.5 hours further, and Maheshwar is about 90-95 km from Indore, so it is a natural bridge back towards the airport side of the state.
Mandu goes last for a practical reason. It is the stop with the most walking and the most to see, and it is also the one place from which a return to Indore is a clean leg: about 95-100 km and roughly 2.5 hours. Ending at Indore means your flight or train home is never far away, and a night in the city gives you a buffer if a drive runs late.
Operators selling this circuit tend to choose the same overnight points: Ujjain, Omkareshwar, Maheshwar, Mandu and Indore. Day-by-day plan
Days 1-2: Ujjain
Land at Indore, drive to Ujjain and check in. Use the evening for the Mahakaleshwar Temple, the anchor of the city. Keep day 2 free for the rest of Ujjain's temples and for resting after the journey, because the next three days involve long drives. Ujjain is about 140 km north of Omkareshwar, according to Wikipedia, which is why the first morning after Ujjain is the long one. For darshan arrangements at Mahakaleshwar, check the temple's current official information before you travel.
Day 3: Omkareshwar
Leave Ujjain early. Allow about 3-4 hours for roughly 140 km; our own guides say 3-3.5 hours, while MP Tourism's itinerary allows 4, so plan for the longer figure. Omkareshwar stands on Mandhata island in the Narmada River, in Khandwa district. Wikipedia describes the island as about 4 km long and shaped like the sacred syllable Om, with two shrines: Omkareshwar on the island and the Mamleshwar Temple, also called Amaleshwar, on the south bank. Plan to see both. Walking the island circuit, the parikrama, is about 7 km and takes roughly 2-3 hours according to our 4-day guide, so keep the afternoon free for it. Stay the night here.
Day 4: Maheshwar
Maheshwar is about 65 km and roughly 1.5 hours from Omkareshwar, so you can see the island in the morning and still arrive for the afternoon. The town lies on the north bank of the Narmada in Khargone district. Wikipedia credits Ahilyabai Holkar with building and embellishing it; her legacy is visible in the ghats and in the Ahilya Fort, which is now a heritage hotel. The town is also known for the Maheshwari saree, so a visit to a weaver's workshop fits naturally into the evening. A single night is enough for the ghats at sunset and a morning start for Mandu.
Day 5: Mandu and back to Indore
Leave Maheshwar early because this is the longest sightseeing day. Mandu in Dhar district is on a plateau at about 633 m, according to Wikipedia. Its best-known monuments include the Jahaz Mahal, Hoshang Shah's Tomb, which Wikipedia calls India's first marble structure, the Jami Masjid, Roopmati's Pavilion and Baz Bahadur's Palace, with other palaces such as the Hindola Mahal nearby. After the sightseeing, drive to Indore for the night: about 95-100 km and roughly 2.5 hours. You can also stay overnight in Mandu, which turns the end of the trip into a slower finish, but you should then budget for the same 2.5 hours on day 6.
Optional day 6
Day 6 is the departure day. If your flight or train is in the afternoon, use the morning in Indore. If you want to stretch the trip, the spare day can go to a second night at Mandu or Ujjain, but we would not add more driving to a plan that already covers several hours on most days.

Reconciling the numbers
The Maheshwar to Mandu leg is the one where sources differ. Our own day-trip guide gives about 39 km and 1-1.25 hours by way of Dhamnod. Other aggregator pages quote about 38-45 km, while MP Tourism's Jahaz Mahal page says Maheshwar is 65 km from Mandu. The difference probably reflects different routes and different start points in each town, but we have not driven all of them, so we give a range and recommend that you confirm the route the day before. For the same reason we avoid a single figure for the full loop.
How much time Mandu needs
Mandu is spread across a plateau, with a fortification wall that Wikipedia puts at 37 km with 12 gates, so you cannot see it in an hour. MP Tourism's Jahaz Mahal page suggests one to three hours for that monument alone. Because Mandu sits between two lakes, Munj Talao and Kapur Talao, on that page, the Jahaz Mahal is the easiest place to orient yourself first. The Khalji dynasty ruled here from 1436 to 1531 according to Wikipedia.
Visiting hours and entry
At last check, MP Tourism's Jahaz Mahal page lists opening hours of 6 AM to 7 PM and an entry fee of Rs 5 for Indians and Rs 100 for foreigners. Fees and timings change, so confirm before you go. With an early start from Maheshwar and a 7 PM close, there is room to see the main group of monuments and still reach Indore at a reasonable hour, but only if you leave Maheshwar early.
Best season for the loop
MP Tourism's Jahaz Mahal page gives October to March as the best season for Mandu, and Wikipedia says Mandu is best visited in the monsoon for its waterfalls. These two sources point to different seasons, so the choice depends on what you want: comfortable walking weather or a green landscape. We have not invented weather statistics for the other places on the loop.

What to skip if you are short of time
If you have only four nights, drop Mandu or compress it into our day-trip guide, and keep Ujjain at one night. If you want to keep Mandu but lose a day, merge Omkareshwar and Maheshwar into one day by starting early from Ujjain; the leg between them is only about 1.5 hours. If you are short of energy rather than days, skip the full island parikrama at Omkareshwar and visit only the two main shrines. Do not skip the overnight buffer in Indore: a delayed drive on day 5 is far easier to absorb when your departure is the following morning.
